Welcome and thank you for choosing our office as your healthcare provider. We are committed to providing you excellence in women’s healthcare. It is important to us that you have a clear understanding of our financial policy. We are dedicated to providing you with the best care while having flexible payment options. Payment of your visit is regarded as part of your overall plan. Therefore, we have adopted the following policies.

Fees and Payments

Fees are standard and based on the complexity of your visit. It is very important that we have a current copy of your insurance card at every visit and keep us informed of any changes in your personal information. This will allow us to correctly bill your insurance.

As a courtesy to our patients, we will check your insurance benefits and make you aware of all costs associated with your office or surgical procedure. In addition, we will verify pregnancy benefits to keep you informed of the specifics of your plan.

While filing your insurance claims is a courtesy that we provide to all of our patients, all charges associated with your visit are your responsibility. Your insurance is a contract between you, your employer and your insurance company. Each visit will be documented in your medical record then a diagnosis will be made by your provider. Each diagnosis is based on medical information, not on coverage by your insurance. In some instances, some services provided may not be covered by your individual policy. Non-payment or rejection of your claim does not relieve you of your financial responsibility.

Self-Pay Patients

We understand the various needs of our patients. Sometimes situations exist when our patients find themselves with limited insurance coverage. Therefore we offer a 20% discount off our standard fees. Payment arrangements can be arranged with our billing office.

Medicare and Medicaid

We gladly accept Medicare patients to our practice. Medicare requires that an Advanced Beneficiary Notice (ABN) be signed at every visit. This form explains which services Medicare may or may not cover and may be your responsibility.

Our practice readily accepts patients with Medicaid, Caresource, Molina, Buckeye, United Healthcare Community Plan and Paramount. It is very important that you keep our office informed of your current eligibility at every visit, as well as any changes to your contact information.

  1. Current Copy of Insurance Card
  2. Current Picture ID
  3. Verify Personal Contact Information
  4. Payment of co-pay
  5. Payment of Outstanding Balance

 Insurance Providers

We accept dozens of medical insurance providers, including most northwest Ohio’s major health insurance plans. If you have medical insurance, we can help you receive your maximum allowable benefits. Some insurance plans may require that you obtain a referral from your regular doctor (PCP) before you visit us.
